Leaving the premises
Students under the age of 16 will not normally be allowed to leave the premises without an adult. If you consent to your child under this age leaving unaccompanied, please tick here.
Medical consent
If, in the event of an accident/illness, and attempts to contact any of the people listed above have failed, we will act in the best interest of the child by authorising any medical attention necessary. This includes blood transfusion and anaesthetic.
Photography
Slough Music Service occasionally takes photos and audio/video recordings of pupils for archive purposes, teacher training and use in promotional material (inc.internet). Do you give your consent to us using images of your child for these purposes?
